Alyssa went to the ER 06.04.2026 for severe oophoralgia. CT scan unremarkable. Suspected cyst rupture, investigation for endometriosis prompted. She would like to explore possible cause with labs.

Alyssa is 14 months postpartum from her first pregnancy. Her iron was chronically low through pregnancy and into postpartum. FE 74, FERR 35 05.24.2025, recheck.

06.11.2026 Abdominal US ordered, CBC, CMP, Lipase, H. Pylori Antigen test ordered by GI.

Alyssa experienced the same pain again with her cycle (day 4) today 07.01.2026. She was on her way to the ER, but the pain stopped and she went home. She noted that both times she visualized a large clot and lots of blood within an hour of the pain onset. Clot size is larger than a half dollar.

Follow up with gyno recommended for abnormal pain and bleeding.

Recommend Vitamin D liquid supplementation as priority to address low Vitamin D. Magnesium spray stack with magnesium glycinate to combat possible poor absorption.

Recommend nutrition tracking ~7 days and audit for macro and micro nutrient assessment.

Recommend follow up with GYN for: bladder (full after urination per ultrasound, incomplete elimination), query regarding pelvic floor physical therapy, extreme cyclical pain/endometriosis investigation.

Recommend follow up with GI for: gallstones, gallbladder treatment plan, extreme cyclical pain (potential bowel involvement), occult blood (2) found in GIMAP results, above range elevated Eosiniphil Activation Protein.

Recommend alternating Tylenol/Ibuprofen if desired in an effort to blunt cyclical pain during upcoming cycle.

Verbal check-in with Alyssa: currently supplementing with magnesium stack, liquid Vitamin D, bone broth daily. Recommend two pumps of Vitamin D every other day. Alyssa reports that alternating Tylenol/Ibuprofen helped with cyclical pain, though she did still feel it to a lesser degree on CD4. She noticed a link to the pain and her bowels this time, and will take this info to GI and GYN. GYN reports that her ultrasounds showed a cyst on her left ovary, otherwise unremarkable.
